When all is said and done—and, yes, there is a bit more saying and doing to endure, which means that anything can happen—the health care reform legislation that President Barack Obama now seems likely to sign into law, while an unlovely mess, will be remembered as a landmark accomplishment.

The bill making its way through the Senate by the slimmest of margins is imperfect, to say the least. But before listing its many flaws, let’s consider the measure’s one great virtue: For the first time, we will enshrine the principle that all Americans deserve access to medical care regardless of their ability to pay. No longer will it be the policy and practice of our nation to ration health according to wealth.

When you blow away all the smoke, that’s what this fight is about. The Senate bill lacks a public health insurance option, the House bill is burdened by gratuitous abortion restrictions, and the final product of a House-Senate conference will probably have both those failings. But once the idea of universal health care is signed into law, it will be all but impossible to erase. Over time, that idea will be made into reality.

The loose ends are so many and varied, in fact, that it will probably be necessary to revisit the health care issue sooner rather than later. Even if it takes years to get it right, eventually is better than never. History suggests that major new social initiatives have to be perfected over time—and that basic entitlements, once established, are rarely taken away.


Progressives who argue for killing the Senate bill and starting over should explain their position to the 30 million Americans without health insurance who would be covered under this insufficiently progressive legislation. They should recall that when Obama and the Democratic leadership in Congress began this crusade, public opinion was solidly in favor of reform. With polls now showing widespread wariness, with Republicans having confused and frightened many voters who already have adequate health insurance, why would anyone think that beginning from scratch is likely to produce a more progressive result?

So this isn’t the end of a process that leads to a rational, sustainable, more efficient health care system. It’s the beginning. But when a reform bill passes, as now seems likely, Obama and congressional leaders will have achieved a goal that progressives have sought for decades. They will have established that quality health care should be for all, not just for those who can afford it.

We have a system now in which Americans go bankrupt trying to pay doctors and hospitals to keep them alive. When you have the opportunity to change this, you take it—even if it means winning ugly.

1. It's also what Tom Harkin
keeps saying, moving from health care as a PRIVILEGE to health care as a RIGHT. Damn important!

But I will probably refrain from eating any sausage for quite some time to come :-).

2. As more details of the bill come to light...
...it looks better & better. There are a LOT of really good things in this bill that have gotten lost in the public option/abortion/taxes/death panels, etc. discussions.

5. Thanks Babylonsis. I rarely come to DU anymore because
of the vitriol being constantly spewed. I guess I better stop here and not read anymore threads for the day. I don't want the constant negativity to overwhelm the positive crumbs we get from time to time.

Eugene Robinson is always the sharpest guy there. I didn't really know much about him before he started appearing on Countdown -- now his column is required reading.

10. We have about as much chance
of changing this health care bill as we have of changing NAFTA - it further locks in control by insurance companies.

There is no way they would give up their stanglehold on the public down the road.

14. Easy to explain to the 30 million -- or better yet address their problems
Do things now that would help immediately.

Expand Madicaid. Implement the "subsidies" right away. Slap on some price controls to bring rates more into reason now.

Put in a non denial of coverage law without mandates NOW -- and if necessary pay the fucking insurance companies to cover the added cost (with a sunset clause) whikle we figure out a better way.

There are many things that could be done to immediately expand coverage and make it more affordable -- without tossing in a lot of poison pills.

Private insurance and pharma are happy to have people go bankrupt paying for care. Private insurance tries to block legislation designed to prohibit rescissions.

This bill takes the powerful pharma and private insurance industry and makes the more powerful. It does it by expanding their customer base and placing few weak restrictions on them.

Meaningful health reform would not only cover everyone and control costs, it would also restrict the power of private insurance and pharma so that we can put the interests of the public first rather than corporate interests. You do that via reimportation laws, prescription price negotiations, a public option. All of these would make pharma & private insurance weaker by reducing their profits and market share. And as a result all were gutted.

So now when we have to redo health care in 10 years, we are going to have to do it with private insurance and pharma companies that are even more powerful and wealthy than they are now since their client base has been expanded, competition has been destroyed and regulations mostly gutted. Health care will be 5 trillion in a decade, and pharma or private insurance will have a lot of that wealth. And they will use it to block any restrictions on their behavior.

This bill is better than nothing, but it does empower some amoral industries by blocking them from competition and regulation.
