1. Mine does and I do.

I suggested last week that they fire my manager after her response to an OSHA violation (failing to enforce proper footwear) was and I quote: "I don't give a f$%k."

I mean we work with food and boiling liquids, she shouldn't be wearing Crocs to work.

Before that I suggested a policy change where partners tasked with training new hires bear responsibility in that trainee's success.

2. Not a chance, because I wouldn't trust promises of anonymity.

I know of a place which has an anonymous suggestion box, but the only way to get away with it is to print off your suggestion at home, fold it up wearing gloves, and drop it while your head is away from the camera. If you use your own handwriting, it will be compared to your job application if the suggestion isn't well-received. If it is printed on an office machine, you will be found out.

Don't even get me started on anonymous polls and surveys. They're as anonymous as I am Chinese.

4. One place I worked did.

And I got blamed by my supervisor for something that was put in there, about him, that I did not do.

But in the end it made him talk to me, and he found out I am not the person he thought I was.

There is so much shit that goes on here that is unjust, unfair, bigoted and uncaring. And I work in a hospital. :(
