Question for those who speak German (especially those living in Germany)

My mom is planning on attending a concert at the rebuilt Frauenkirche in Dresden, as she is a member in an organization dedicated to its restoration.

Unfortunately, she doesn't speak enough German to understand the payment instructions and form that were sent to her. We've tried e-mailing the people who are selling the tickets, but have received no response.

Does anyone have any suggestions as to how we decipher the form that was sent?

those things are pretty easy to figure out if you know some Deutsch.

My mom has the form at her house. She doesn't have a computer; but I'll ask her to bring the papers by tomorrow. I think she only needs to know where she should put her credit card number; but there's something on the form that refers to a bank number, so it's a little confusing.
