The payments left nothing for even the $65 co-pays much less anything else medically related.

I don't think a good segment of folks have any idea about not having $65 on hand at any given time and with cost sharing increasing people may be required to come up with significantly more than that.

Insurance you can't afford to use is a "Death Panel" of one. You don't use unless you think you're going to die and even then you might have to borrow some of the price of admission, if you can.
