I meant the series of events leading to a disaster. I've seen a documentary on Piper Alpha and from that I know that the key issue was "Claymore and Tartan were not switched off with the first emergency call" They were the incoming pipelines.
Yes the North Sea is cold. One of the guys on Piper Alpha jumped from the deck down into feeling he had no alternative. Miraculously he survived.
