The fire started when a Zuni missile cooked off of an aircraft to the rear of McCain's A4. It hit his aircraft and started the whole mess. At that time, the Navy did not have thermally protected bombs. As a result of the Forrestal fire, the Navy developed thermally protected bombs. They had been dropping WWII vintage bombs from carrier aircraft since WWII.
