.. is worse than any epithet. It doesn't matter what word you use, people are still going to have a stigma attached to it.

I don't find nutcase or wacko particularly offensive. Maybe because it really doesn't carry the same emotional weight as "gimp". Gimp implies uselessless, lump of flesh, with bits of Nazi-esque overtones thrown in, "life unworthy of life". "Wacko" or "nutcase" doesn't.
