Librarians, teachers, parents -- recommendations for 4th/5th grade reading level books?

trying to think up some book ideas for my younger daughter. She's 11 but has a learning disability and reads at about a 4th grade level. I'm looking for something that will challenge her without frustrating her. It seems like all Scholastic offers these days is TV and movie tie ins and I'd prefer not to go that route.

Thanks in advance for any suggestions.
