I was reading Maher's official message board, and this is from someone in the audience:

Tonight's REAL TIME was a great experience, especially being in the studio audience. I'd never been at a LIVE tv show, and the audience, by now knowing that Michael Moore was a guest, was fully in the zone. Upon watching the 11PM tape-delay here in L.A., this was more than apparent. The audience, as Bill mentioned in the top of the show, was transfixed by Moore's "ROCK STAR" presence, and, therefore, we ate up everything he said. This was no more apparent than during Moore's silly "What if this building was under attack?" exchange with Dave Dreir.

Dave Dreir did, in fact, storm offstage during "New Rules," and, I think specifically during the "thumb gesturing" gag that Bill pulled....Dreir did his own share of thumbnastics during the show, and it must have been the straw that broke the camel's back.
