Consider this - I don't think Strom Thurmond could even function the last 12 years in office and they kept him in office.
But the general policy is that the governor will appoint a replacement which will probably come from the party of the governor. Then in the next major election in South Dakota a special election would be held to fill out the term of the senator.
But let's not put the cart before the horse, people have survived strokes
