It's a 45-second ad, not a documentary. The point is to trigger the conversation and coverage. This does that. You can bet that by tomorrow, all of the networks will be showing the ad , discussing his impairment and augmenting the discussions with numerous clips of the kind you're talking about.
And if this reminds them of their elderly relatives, great. They don't think Trump is elderly or shaky and definitely don't think he's like their elderly relatives. He sells himself as vibrant, youthful and the healthiest specimen of a man anyone ever knew. If this makes them think he's like the old, infirm people they know, that's a pretty humiliating comedown for him.
