Texas is so Covid deficient, I don't believe the leadership in Texas
even cares about vaccine cards.

California uses a QR Code, so when it is scanned it looks like the above.
It requires a 4 digit code and only sends it to your personal phone.
If someone hacks your 4 digit code, only your phone will receive the QR Code.

Heck, if you print it out and stash in your wallet, you're good to go, no?

I double-dog dare anyone to figure how to forge a QR code

They set this up months ago, so I'm guessing that they've worked out the kinks. I accessed mine today to download a copy: it took about 5 minutes. You can transfer the pass to your smart phone, or save a copy to your computer to be printed out. It's QR code-based. (The venues requiring it are expected to compare it to your NYS ID or driver's license.)

For some unclear reason, NYC recently decided to create its own version, even though the master vaccination database is maintained by NYS. NYC residents can use the Excelsior Pass rather than that one if they choose.

finally was found after 5 submissions, talking to them by voice (all they can do is direct you back online, there is human to engage), and talking to the medical group I got my vaccine from. They showed it was submitted and they did not have the ability to resubmit.

CA =finally= after over a month found it by some surprise magic.

get a card filled in to show we'd had the first dose, which we hadn't been given.

At that point the federal and state offices (vaccination performed by FEMA) referred us to our county of residence (thus supposed to be of vaccination), which did turn out to be keeping the records at that time but didn't seem to know it and kept referring us to FEMA and state offices.

Whatever, official vaccination records are kept in FL. Somewhere. Noting that the Republican governor has banned anyone from requiring proof of vaccination or vaccination "passports."

both set-up and retrieval. At no time are you entering information such as social security numbers, but they do digitally image/confirm your face against your driver's license on your first registration. I would expect the others are doing something similarly secure as well.

The CO app also lets you upload other important frequently need documents as your car insurance, registration, and useful for me, professional licenses.

I would think that the VA reported to the state.
Go to the WA. link and follow whatever directions and see if you're record is listed. WA. uses https://www.MyIRmobile.com

.

It not only verifies your ID, it directly ties your ID with your vaccination status.

There is no HIPAA exposure to state you are vaccinated. But, allow people to Opt-In as a courtesy.
