(this is the first time I might find the term "exquisite pain", actually appropriate, :) ), he is on some pretty powerful analgesics; I would think Demerol, morphine or dilaudid. This opens up a whole bunch of options. He might actually have lucid thoughts about being the unbelievable creep he is.

But I would suppose, he is looking for new items to expose the evil demons that possess everyone but his inner circle. As usual with these people, their demons are a lot closer to home than they would suppose.
His remnants of a soul would look like a shriveled black prune, waiting to be plucked and returned to the seventh ring hell from whence it was spawned.

In the mean time, I hope he enjoys the benefits of modern medicine; something he is more than willing to deny some 40+ million people across the nation; they can just rely on someone with a skull on a stick and dressed in a grass skirt.
