1. Well if bush was incapacitated
Cheney would automatically be the president so I guess it would be Cheney vs. Kerry. If Kerry were to be incapacitated, the Dems would probably get behind Edwards, as there is no time to nominate banyone else.

4. Bush has been incapacited for four years and that hasn't....
...prevented him from holding the highest office in the land

6. Mel Carnahan was "incapacitated", and remained on the ballot and won.
Would it be the same?

7. No
Carnahan was just a private citizen when he ran for the Senate. If a president becomes unable to perform, the Vice President takes over. It's in the constitution. Not really sure about Missouri law, which is where Carnahan lived.

9. Carnahan was Governor
and the state Democratic leadership decided there was insufficient time to replace him on the ballot, so chose to keep him on it and just campaign with the fact that his wife would be appointed if he was elected.

The Constitution has absolutely nothing to do with party nominations.

8. The RNC or DNC leadership would meet
and choose a successor for the nomination.

Just because Cheney would become president doesn't mean he is automatically the repub candidate.
