Click Start then Programs then Microsoft Works. Click "Programs" at the top. When that page opens you should see several templates one of which is "Financial Worksheets" Also try clicking "Templates" at the top.
There may be one of the many templates available that you can modify for your purposes or, if you have the patience, you can construct your own using that program.

On edit to add a BTW...
If you have several investment accounts like at Fidelity and elsewhere, it is a good idea to do a beneficiary review to make sure that they all agree. It is not uncommon for beneficiaries and contingent beneficiaries to vary from account to account, especially if it has been a while since you opened the accounts or if it was a long time between opening accounts. You might have simply placed names in a different order, for instance or named them "Per Capita" instead of "Per Stirpes" or maybe now there are grand kids when there weren't before, etc.
Also it is not uncommon for someone named as a beneficiary - say 10 or 15 years ago to perhaps fallen out of favor or maybe they don't need or don't want to be named. Doing such a review every so often is a good idea.
