It happens sometimes when you get your car serviced, boxes ticked but whatever got ticked didn't actually get done.

Workers under a lot of pressure to get stuff done in insufficient time allowed will do that, look for shortcuts.

Doing that during aircraft assembly though, is serious stuff.
