1. The insurance business should have nothing to say about health care.
That's the "reform" I want. They can sell insurance for health care costs if they want to, but otherwise they should have no part. The health care system should be a public service created and managed by the various appropriate governments.

3. Canada did it province-by-province
starting in those socialist prairie provinces.

It may have to go state by state here too. To some extent, the way is being cleared for that with the waivers.

5. I think we've allowed the cancer to become too pervasive
and probably will have to go full on NHS to ever get a handle on costs again.

Single payer would help by eliminating the leech like middle man from essential coverage but the profit motive has greatly perverted the system.

I also think we need to wrap up all care into one package. Mental health, vision, and dental need to be folded into major medical badly.
Hell, just that adjustment would be more meaningful than what we passed to most folks.

6. As you can see "it's not that simple" but obviously a single payer system is the only way to cover
everyone.
They will also have to regulate the costs of procedures and medication.
One won't work without the other. And that's easier said than done, especially with the drug makers throwing jillions of dollars at the lawmakers.

7. The only chance we have of doing that
is if America sees the stupidity of electing either Republicons or Blue Dog Democrats to Congress, and puts in some solid progressives. That can't happen until next year's election at the earliest, and perhaps a President Obama who will no longer have to face another election will have the courage to lead that new Congress on this issue.

I wouldn't hold my breath on that scenario happening anytime soon, unfortunately. We'll probably dither away another decade before taking a serious look at the problem again, and having all of the pieces in place.

I'd rather deal with a government bureaucrat who doesn't care if I die than a corporate one who's livelihood depends on it. I think I'll change the word one to bureaucrat to make it just a little more effective.
