The President pro tempore is the most senior senator in the majority party.

3. Only by tradition.
If he wanted, he could decline the offer. Of course, it's purely ceremonial unless there's a disaster that wipes out the POTUS, VP, and Speaker all at once.

The current President pro tempore of the Senate is Robert Byrd (D-West Virginia). He replaced Ted Stevens (R-Alaska), who held the office from January 3, 2003, to January 4, 2007.
