Even Jimmy Carter, bless him, knew what was going on, he just treated it naively. Ronald Raygun even knew about AIDS, he simply wanted gays to die off. The fact that no one could tell what was going on in his head did not mean that he didn't approve of Iran Contra (he did), that he did not want to avoid congressional bans on Nicaruaga (he did, and order North to do it) that he did not realize that scaring USSR would have the impact it did. (He even joked about it later).

Reagan was much more aware than Bush, and even he never had to claim he was on top of his own administration.
