I'm going with the paper/podium theory because that means one of two things:

He's constantly leering at her ass and no one has told him to knock it off, or
He's constantly leering at her ass and someone has told him to knock it off and that is the issue he decides to get all "Maverick" about and says, "Screw you! I can stare at her ass in front of hundreds of people and news camera's all I want because when I was a POW, I didn't have a woman's ass to stare at and I'm a Maverick!" (then after that really long run-on sentence, he'd need a nap.)

McCain really has an issue here. I just don't buy it that the handlers wouldn't have gotten rid of the perception if McCain was just staring at a piece of paper. He did it worse today than he did on the initial appearance.
