"By endorsing and cashing this check I authorize XYZ Company to switch my telephone long distance provider to Gowjem Telco."

"By endorsing and cashing this check I authorize QRS Company to add my name and address and telephone number to their opt-in feel-free-to-call-me-anytime-day-or-night-I-love-telemarketers database."

of crossing out the terms and just endorsing the check. Seems that would probably hold up. Any legal minds?

Many times there are boxes in pizza places or laundramats for entering contests. And by submitting your name, you're authorizing some company to switch your long distance service.
