1. In my department, we have a lot of no-shows.

Sucks because there is a waiting list of a month or more for the tests we do.
I wish we could charge a no-show fee, but we can't.

I'm sure if a patient had a good reason for not showing up or cancelling at the last minute, the fee could be waived.
A lot of people just don't want to come because they are scared or it's unpleasant.
