Intersexed individuals generally have something odd in their chromosomes, or they were born hermaphroditic at birth, and were 'assigned' a gender arbitrarily by their parents and doctors when they were infants.

Trans folks may or may not have any physical reason for feeling that their gender is incorrect, but feel nevertheless that they are trapped in the wrong skin.
