He trusts her and stuff...
She explained she'd let her friend and some guy use her bed. But the friend it was hasn't said anything about it to him... not even an apology for causing him a bit of stress.
That, of course causes him more stress.

The fact that she didn't means that the wrapper was hers. (Of course, it could also mean that she has really low hygiene habits, which is also a valid reason to break up with her.)

Your friend sounds like he's in denial. He says he trusts her, but wouldn't be stressed, and wouldn't have even told you, if he really trusted her. He knows, he's just hoping it isn't true, and that someone can convince him it isn't. (Been there, done that).

Question is, why was the wrapper left? If she left it, either she doesn't respect your friend enough to care if he gets hurt, or she wanted to get caught. Or it could be the guy wanted her to get caught, and dumped it there.

There's an outside chance she's telling the truth, but it seems like your instincts and your friend's say otherwise.

and ask if she had used the bed for such (not mentioning the wrapper -- this might predispose her to "cover"). If pressed for a reason for asking, I might say something like: "I dunno, it just kinda weirds me out."

But the friend could already be warned to lie. (That she hasn't said anything can be read multiple ways -- including that she'll lie if she has to -- but she won't go looking for an opportunity to do so... a common enough half-way stance.)

If your friend's girlfriend is cheating, she's doing so stupidly.

I would not assume that she cheated on him. But there is fairly decent circumstantial evidence that she did. There is, however, virtually none that she is actively cheating on him.

It is not all that unusual for something to "just happen", as they say, with some guy and never again.

With that guy. I have found that people who cheat once are prone to cheating repeatedly even when they honestly believe they never will.
