Questions about NOV. 2ND in Ohio because I can't search :(

I need links!

In this video,
http://easylink.d2productions.net/votecobb/votecobb05.wvx , a form is mentioned which has to be signed in order for a voter's provisional ballot to be counted. Also, I believe the ballot itself is supposed to be signed by a precinct official in order to be valid.

My questions are:

1. Why didn't the polling places have the forms on hand, and were there any Blackwell 'decrees' that had anything to do with this? Was it just lack of information? IS this something Blackwell was warned about but neglected to inform citizens about it?

1a. Was the need for a precinct official to sign the provisional ballot printed on the ballot itself?

About redistrcting/precinct changes:
I understand Blackwell made these changes 2 weeks prior to the election. I read that some colleagues or state officials trie to tell him that he needed to inform citizens of new districts but Blackwell ignored these requests.

1. Who tried to tell him/ who did he ignore?

1a. What were his reasons?

About the voting machine shortages:

I read that there were some perfectly useable machines locked away in basements or wherever on Nov. 2nd. Despite many phone calls to Blackwells offices from Precincts with long lines who desperately needed more machines, these precints were not helped. What was the story here?

1. Who said no, and what were his/her reasons for not providing these machines to the urban, minority precincts that needed them?

Can anyone give me a link showing the amount of machines in these long-line, urban areas as opposed to the amount of machines in white neighborhoods?

Thanks in advance if ANYONE can help. All I need are links and a few words about what the link pertains to; I don't need any long explanations but I certainly welcome whatever anyone can offer either way.
