I'm guessing here, but I'd bet the biggest reason would be no access to good prenatal care.

And I wonder how "racially pure" they want people to be? The U.S., Australia and Canda all have higher rates of orthodonture than European countries. It's been theorized that the reason for this is the mixing of the various European groups in these three "immigrant" countries...In other words, my big Swedish teeth didn't fit into my petite Irish jaw (or vice versa). I suppose being a mix of Nordic and Celt makes me "impure".
