As crazy as it sounds, the materials are readily available and the technique is not too hard to learn.

Here's a site that explains it step by step (although this site is about making "vampire fangs" it does use all the same dental materials and techniques needed.)

on edit: you should probably get your dentist to install it for you after you've made it, just to be sure it's properly glued.
