You have the Check mark in "Automatically move pointer to the default button in a dialog box"? (Control Panel > Mouse > Pointer options)

Those are the 3 biggies.

Then there is hair and stuff on the bottom of the mouse where the LED is.
In a ball mouse, hair and stuff inside the mouse!

If it's a LED mouse, it could be the surface you are using. You can run them on damn near anything except a seriously monochrome and texture-free surface. Try an old mousepad.
