(CNN)Layleen Cubilette-Polanco was arrested in April and sent to New York's Rikers Island jail because she could not afford the $500 bail, her family said. Nearly two months later, the 27-year-old transgender woman was found unresponsive in her cell and later pronounced dead, according to the Department of Corrections.

Her death is the latest in recent weeks to draw attention to the plight of the transgender community. Civil rights groups say Polanco's death represents a web of factors that can trap people of color in the justice system -- especially transgender women of color -- with devastating outcomes.

"We know that the system failed Layleen," said David Shanies, a lawyer representing the family. "What we're trying to find out now is how that came to pass."

While officials investigate what caused her death, a community is in mourning and her family is searching for answers. They want to know how Polanco ended up at Rikers jail for misdemeanor charges and why was she left alone despite a medical condition that gave her seizures.

Mayor Bill de Blasio promised a transparent investigation of Polanco's death......................

So so much is so wrong. Words fail me. #discrimination

Mourners gather Monday in lower Manhattan to remember Layleen Cubilette-Polanco.

3. We need to know how she died.

New York is one of the wealthiest and most progressive states in the nation. Why is Rikers Island such a shit hole?

6. Just like law enforcement; there is a culture of depravity toward inmates by corrections officers

towards inmates; specifically minorities.

Endemic racism; xenophobia and a culture of white nationalism is rampant in the ranks of corrections centers in the United States.

The death of this inmate is a travesty that is a direct result of a lack of oversight and proper internal affairs to eliminate the culture of violence that is fostered by corrections officers.

Just as there are no good cops; there are also no good corrections officers either. A good decent human being would not allow this to happen. Period.

The “good” cop lost all credibility and has become complicit in this crime when s/he looked the other way while this has been allowed to occur...

11. It's the other way around many times in Rikers

I am close friends with a correction officer there, and he is far from a racist. He's nice to everyone there, whether they curse him out (for no reason) or not. Recently, he was just attacked and badly beaten. He'll be out for many months, blindsided by inmates for absolutely no reason.

16. It's on the official list of DU avatars

Skinner put it there, not me. Jackson has faults, but many merits as well. He practically founded the Democratic Party.

17. Very true

My friend the CO at Rikers was always nice to them, whenever anyone said "you must be a racist to be a CO" his answer is that he joined the job because of the benefits, and that he doesn't judge anyone, as it was not for him to say.

Most of the inmates were cool with him for this, but some didn't care. They saw him as just authority who deserved to die. As if they were the good guys and all cops/CO's were evil.

5. So Evil And Disgusting

Why people have a problem with any of this is beyond me.

Again two simple principles to live by are do unto others as you would have them to unto you and also live and let live.

Hate is evil outside of morally bankrupt people like Trump I do despise him.

9. A transgender woman spoke at the Pro Choice Rally and it was very sad

to hear her experience trying to get a safe abortion. Several speakers brought attention to the community and I am glad they did. I was unaware of how their situation and the danger they are facing.

13. The purpose of bail

is to ensure that the accused shows up for their court date.

She had already demonstrated that she probably would not have shown up for her court date as she missed prior court dates as part of an alternative to incarceration program stemming from prostitution charges.

The court assigned a very low bail amount probably based upon the fact that her financial situation did not warrant anything higher.
