There has to be a process issue (like a polling place running out of ballots... Or equipment problems...)

such as a station ran out of material and they had to send for more. They extend that polling station by that amount of time.

At other times if the line is way too long, they have done that, until all voters in line when they closed vote, not those who arrive later.

2. No, and not necessary. In WI, if you're in line by 8:00, you will be allowed to vote.

So the poll closing is automatically extended for those arriving at the last minute.

They shouldn't. If there is an extension it would only apply to specific locations.

I haven't heard anything to suggest it should happen. And as long as the voters are in line when the polls close they can still vote.
