I'm an MD and I would do everything I could to save his life. It's how I was trained and it was a big part of an important oath I took.

And remember- he's only 19- it could be that his brother lured him in- I'm NOT at all excusing him but if healing his wounds gets him to confess- its well worth it. He will never be free but we can learn much from this lost soul.

Otherwise, how to find out the motivations?

He should be listened to, able to air his thoughts and prejudices, etc., so we can think about them and whether he had a point in his mind, and how we should address it. Otherwise, his "senseless" violence will never find a cause and we will never understand this mindset and how to disarm/defeat it.
