1. apparently, the protocol indicates that one may be addressed by the highest title one has achieved

even if one has been booted out of that office.

if one is talking or writing about a person, rather than speaking to that individual, then it is appropriate to refer to former speaker, or former governor, etc.

Why was corrupt former Police Commissioner Gillian Loeb still called "Mr Commissioner"? Do you know what they called Napoleon when he was exiled to Elbe Island? They called him "that asshole" mostly, but to his face they always said "Your Imperial Majesty." Rejecting good form is rejecting civilization, even if the occasional scoundrel gets an undeserved honorific out of the bargain.
