Unlikely there will be a Federal Passport, and most States won't provide them, so what are you going to do?

You cant get into certain countries without the yellow card showing your vaccines. and yes, its a paper card. The trick is to put teeth into penalties for forging or selling or transferring your vaccine record.

They also have electronic records so that could be an avenue too.

...and myself are all vaccinated. Yet, we still wear masks when indoors because, well, we're adults. And that's what adults do.

Just sign up for an account and have your doctor or health care provider verify your vaccine status and then all places have to do is scan a barcode on your phone. No need to carry additional documents and it's harder to copy or forge.
