1. You have to fill out the form in order to get the rates...and they vary considerably...so it will

take some investigation on your part.

Depending on which state you live in check your state gov site or contact your insurance commissioners office maybe. But plan rates and subsidies depend on income so you will need to provide a rough guess about that
