They are a bottomless pit of self loathing that needs constant adoration to overcome that. But, like a heroin addict, the hit that used to do the job doesn’t do anything so they need more. The narcissist requires you to give up something you value to prove your love to them. No self abasement on your part is too much. But then they push too far and you realize what has happened and you stop, you say no. They respond with incredible rage as they cannot believe you would betray them that way. After all, you said you loved them once, why would that change. They will never, ever see the roll they played in this.
