How does family coverage work under mandatory insurance plans

Let's say a person works full time and their spouse stays home with the children. Will it be mandatory for the employer to provide the employee with coverage or will the employer also be required to provide family coverage as well?

If the family coverage offered by the employer is outrageous, like $800 extra a month, will the employee be forced to buy that coverage through the employer or is there typically a more affordable plan available that they are required to buy?
