1. He didn't screw the barrel down far enough.
The M2 50 cal's barrel screws into the receiver and then backed out a couple clicks to properly headspace the weapon. If you don't screw it in enough or back it off too much, the thing you see in the video will be the result.

Bottom line is that that machine gun receiver and barrel are now deadlined.
