AKO link functioning again

The “My Forms” link of Army Knowledge Online is back in operation for the submission of evaluation reports and other administrative actions following a five-week suspension for system upgrades.

My Forms, a component of the Army’s Forms Content Management Program, can be accessed in the upper right-hand corner of the AKO homepage at http://www.us.army.mil .

Reactivation of My Forms means that non-deployed units no longer are authorized to use e-mail to submit evaluation reports to component personnel centers.

Deployed units can submit reports via any approved means available, such as unit upload, e-mail and regular mail. However, they must use the My Forms system, and its associated “Send to DA” function, when they return to home station.

My Forms (Pure Edge) provides for the electronic preparation and submission of reports for both the active and reserve components.
