About 20 percent of performance evaluations turned in to the Air Force Personnel Center are being sent back to raters because the forms were filled out incorrectly.

In August, the Air Force introduced new electronic evaluation forms for enlisted airmen and officers.

“Most raters filling out the forms are not having problems, but with any new process, growing pains are expected and the new evaluation forms are no exception,” Tech. Sgt. Jason Malec, noncommissioned officer in charge of Air Force evaluations at AFPC, said in an Air Force release about the problems.

Common issues include unsigned forms, blank boxes and wrong file names, Malec said. These errors are easy to correct if caught before they become a matter of record. If left uncorrected, an evaluation appeal package must be filed to change the evaluation

Supervisors with questions about how to complete the forms can check two Military Personnel Flight Memorandums, 07-44 and 07-45 that provide step-by-step instructions.
