Does the tenth house represent fame? And if a person has Saturn and Pluto there, does that mean that it's pretty much a no go?
Also, what does it mean if a person has most of their planets in the upper part of the horoscope? Mainly 10th, 11th and 12 houses? I interpret it that it means they will be more concerned with groups and professional life than personal issues. Is that correct?

I would expect a person with planets in the 10th to be concerned with fame. That could mean famous among patrons of a certain bar, or famous in the world. I would not rule out fame for a person with Saturn and Pluto there. Saturn can indicate a need to conquor the issues of the house, so it could cause a person to want fame more than someone without the placement. Pluto usually stays behind the scenes, but the power in Pluto needs to be released. So these planets could indicate a desire for fame. They wouldn't come across as warm and fuzzy, BTW.
I would say the planets in the upper chart as indicating a person who lives in public view, even if nobody is watching. Everybody is concerned with personal issues. That's how I would say it.

would give an extreme desire for (professional) power and recognition. The person is probably very ambitious. Saturn there would indicate that achievement doesn't come by itself but the person would work hard for it. Saturn is naturally placed in the 10th house - it's comfortable there. It would depend on the aspects and sign placement to determine whether that recognition is achieved - or notoriety. It could also, if badly aspected, indicate difficulty with authority figures.
Most the planets in the upper part of the chart can make a person more outwardly focused.

Tenth house cusp is Libra, as well as Saturn and Pluto (Jupiter too, but it falls into the 11th house). The person I did the chart for is my first Reiki student. He wants to be a singer, but I have a feeling that if he achieves any type of fame, it's going to be for his psychic/healing abilities (a double Sag with 12th house sun and Uranus and Neptune in the first and Moon in Scorpio). He's also a massage therapy student and he did wonders for a crick I had in my neck last week.

if you use Vedic or Sidereal astrology, it would probably put Virgo on the 10th house cusp. That would be more in line with your thoughts about the person's healing abilities. On the other hand Libra corraborates his desire for artistic fame. 12th house sun can be a tough placement, but also indicates a lot of compassion.
