What are the Republican rules if McCain drops out of race? Does Palin automatically become the head?
Or do they do some sort of RNC committee vote to select a replacement?

5. Ballots in many places are already printed
However, you're not really voting for the candidates directly. You're voting for the electors who will go to the Electoral College and select the president. They can pretty much vote for whom they please.

However, if they select anyone other than McCain/Palin -- in that order -- it could result in a constitutional crisis that would have to be decided by -- drum roll, please -- the Supreme Court.
