Unless I want a roomy sweatshirt, in which case I go for medium.

6. My husband wears an 18, or an 18½ if we can find one.

It can be frustrating, because the shirts in that size don't always come in as wide a variety of colors. He wears 34/35 sleeve length.

5. depends on what I need

Large for formal collared shirts or a more tight fitting T-shirt

XL for things like an over shirt or something I want loose fitting.

Kinda a skinny little guy, but I like big roomy shirts. Big enough I can fit someone in them with me, heh heh heh.
