Bosshole of the Week?

I work for a small satellite office of a large company and today my boss called me and 2 co-workers into her office and proceeded to shake and yell and criticize and cry for an evaluation that we supposedly "anonymously" filled out on her back in Nov. She just got the results on Friday. Obviously since we filled these out in supposed anonymity, yet she has taken upon herself to try and attribute comments to each of us, I'm now concerned for my next review from her - per her words. Should I contact my corporate HR office and fill them in?

The weird thing is that on a scale of 0-5, she got a 3.84! It's not like we gave her a 1. Unbeknownst to us, less than a 4 was unfavorable. Not that it would have mattered.

Sadly, a co-worker that was called in with me didn't even get the opportunity to fill an evaluation out on her. They sent it to another co-worker by mistake. She never bothered to correct it because she was so angry and had spent the weekend assuming it was him so he had to sit through it.

I have never much gotten along with her and feel like either way she's working on my demise. Been with the company for 5+ years.
