Buy new: $19.88 Used & new from $15.85

I think I'll fork out the extra 4 bucks for an unused replacement brush, thank you.

is acting as the agent, not the seller. The toothbrush won't be used, it will simply be offered for sale by someone other than Amazon.com using Amazon as the sales agent.
