1) Bush is a control freak and demeaning others helps for him to maintain that control. Schoolyard bullies pull this stunt on the playground; always bringing up a physical difference in a condescending way.

A dog trainer told me that was his way of dominating me. He ran alot of my nylons by doing that and I wanted it to stop so the trainer told me to put my hand firmly on his head when he did it. Whether or not rubbing heads is intended to be offensive depends on the situation. I do it to friends and acquaintances occasionally as a friendly gesture, and it's barely a conscious action.
