The thumb sticks out the side of your hand. If he had the outside of his hand chopped off, including his little finger, and had thumb bent into his palm tightly, it could be his index finger, but no way for thumb. Take a picture of you doing this with your thumb and post it and I'll believe it.

The top of the hand, that the "protrusion" is sticking out of, is just too wide to be a hand turned sideways (with the thumb up).
If the protrusion is a middle finger, the shape of the hand at the top makes more sense.
