A BBC reporter killed during a Taliban assault on a television station was in fact shot dead by US troops trying to clear the building a military investigation has confirmed.

Ahmed Omed Khpulwak was killed in a case of mistaken identity by a soldier who feared the reporter was a suicide bomber.
The conclusion follows an independent study which suggested Mr Khpulwak had been shot dead with US-made weapons while he hid in a bathroom, after the Taliban attack was over and the insurgent attackers had already blown themselves up.
Mr Khpulwak, a reporter with the BBC Pashto service who had also done freelance work for The Daily Telegraph, was among at least 22, mostly civilians, killed during a militant attack on government buildings in Afghanistan's Uruzgan province in July.
The investigation concluded: "He was shot by US forces who believed he was an insurgent that fired on them with a weapon and was subsequently taking action to detonate a suicide vest improvised explosive device (IED)."
