Someone bleeding like that can be admitted overnight for observation (fluids, whatever) until the specialist is in the next morning.
I was married to an internist from med school through attending, and I can tell you that, if any resident in his program did this, there would be hell to pay. Residents who are in over their heads (happens often) call the attending. If the attending refuses to come in (happens rarely), then he calls another one and documents it in the chart for the ER chief to deal with.
