It doesn't seem that this would be normal gear for a marlin fishing outing. So it appears that he may have had some other activities in mind or maybe he just happened to have overlooked it while packing. I doubt that he even does his own packing either, so it could have been intentional.

Also is it legal to have a doctor prescribe a drug in his name to be taken by a patient as I understand what happened. The whole thing smells fishy. Pun intended. I don't think my wife would be too understanding if I went on a fishing trip with my buddies and packed Viagra along with my gear. That one would be HARD to explain away.
