And for a Health Care Advocate (Health Care Power of Attorney) - June of this year. They provided no counseling, but recommend that all patients fill out the papers. I already had my forms from previous years, so provided copies of those. When I had surgery at the same facility in January 2007, they did not include this paper work in their packet.
